- Application addressed to the vice-rector for scientific and medical work;
- Copy of passport;
- Personal sheet for personnel records;
- Autobiography;
- Characteristic - recommendation;
- Photo 3x4 - 3 pcs.;
- A copy of the diploma of graduation from a higher educational institution;
- A copy of the certificate of residency graduation from completion (for clinical departments) or a diploma of graduation from the magistracy (for theoretical departments);
- List of published works and research reports. Persons who do not have published works submit scientific reports (abstracts) in their chosen specialty;
- Extract from the department meeting, petition;
- Extract from the work book;
- Certificates of passing candidate exams (if any)
- Letter from the prospective supervisor with consent to the scientific guidance and the proposed topic of the dissertation.
